SPECIALIZED CARE

Skilled Care 24/7

SKILLED. CARING.

Dedicated.

Many of our residents have complex medical needs that require 24-hour skilled nursing care. Our highly skilled team of RNs, LPNs, CNAs, DSPs and Case Managers is dedicated to providing the quality services residents need in a supportive, home-like environment.

A Personalized Approach

Because of residents' limited ability to voice their care concerns, our nursing team constantly monitors and manages the health of each person in our care. We strive to develop effective relationships with residents and their family members to identify care needs and develop personalized care plans.

Our comprehensive service is designed to meet the habilitation, restorative and long-term needs of individuals who are victims of genetic disorders, birth trauma, accidents, physical abuse and/or debilitating or life-threatening diseases.

Our clinical services and treatments include but are not limited to:

Respite Care

Caring for a loved one with a developmental disability is a full-time job. It's important that you take respite breaks for your own health so you can bring your best self to caring for your loved one. At Swann Center, we offer respite care for a few hours up to a few days. If you need to take a break, we'll ensure your loved one receives the very best care. For more information, contact us.

The experience challenged my preconceived notions about disabilities and the available support services. I realized that individuals with disabilities possess diverse talents and abilities, and with the right resources and accommodations, they can actively participate in various activities and contribute to their communities.
